Sedation dentistry is not a new practice. General anesthesia, which relieves both anxiety and pain, has been used in dentistry since the 1840s. Today, sedation dentistry is used to relieve anxiety, when needed, in conjunction with pain relief from local and/or general anesthesia. How will I know if my dentist is qualified to perform sedation dentistry?
The American Dental Association's (ADA) 17-page "Guidelines for Teaching Pain Control and Sedation for to Dentists and Dental Students" outlines minimum requirements for training programs.
What courses are dentists required to take to be "sedation dentists"?
Upon completion of a course in moderate sedation, dentists must demonstrate competency in:
- Advantages and disadvantages of moderate sedation dentistry.
- Prevention, recognition and management of complications associated with moderate sedation.
- The technique of intravenous access, intramuscular injection and other parenteral techniques.
- The pharmacology of the drug(s) selected for administration.
- The precautions, indications, contraindications and adverse reactions associated with the drug(s) selected.
- Complications associated with techniques of moderate sedation.
- Prevention and/or management of emergencies in the dental office.
- Principles of advanced cardiac life support
- The abuse potential, occupational hazards and other untoward effects of the agents utilized to achieve moderate sedation.
- Historical, philosophical and psychological aspects of anxiety and pain control.
- Patient evaluation, physical diagnosis and psychological considerations.
- Physiological and psychological aspects of anxiety and pain.
- The distinction between the conscious and the unconscious state.
- Pediatric and adult respiratory and circulatory physiology and related anatomy.
- Pharmacology of local anesthetics and agents, including drug interactions and contraindications.
- Indications and contraindications for use of moderate sedation.
What type of internship must dentists complete?
To meet training requirements, dentists must:
- Administer moderate sedation to patients in a clinical setting safely and effectively.
- Administer the selected drug(s) to dental patients in a clinical setting safely and effectively.
- Demonstrate the ability to manage emergency situations.
According to the ADA:
"Moderate Enteral Sedation Course Duration: A minimum of 24 hours of instruction, plus management of at least 10 adult case experiences by the enteral and/or enteral-nitrous oxide/oxygen route are required to achieve competency;" and
"Moderate Parenteral Sedation Course Duration: A minimum of 60 hours of instruction, plus management of at least 20 patients by the intravenous route per participant, is required to achieve competency in moderate sedation techniques."
